Dr. Scott and Dr. Percell are accepting new patients. Dr. Endo and Dr. Farley are not taking new patients with the exception of new OB (pregnant) patients, immediate family members, and direct referrals from current patients. - Health History (for patients 18 and over)
- Conditions of Patient Registration
- Information Release (optional)

This form needs to be sent to your old doctor who will then forward your records to our clinic
